Here, parents are given an oppurtunity for their own input. Two questions on this page asks for a parent's thoughts on the student's educational program and to describe the student's strengths, interests, personal accomplishments and individual attributes. ​


Finally, a third section of this page makes a statement describing how the student's identified exceptionality (again, coded as "disability") impacts their ability to participate in the general education setting.​
